Modified Usage of Monofixators



Abstract
The fracture pattern of extremity is quite variable and it could combine varying degrees of soft tissue injuries.
Most open fractures of long could be manged by the standard application method of monofixators. But, in some cases such as metaphyseal open fracture which could involve neighboring articular surface or has some bone defects, modified method of
application of monofixator is quite useful.
Authors experienced both standard application and modified application of monofixators.
We report the 35 cases of modified application of monofixators.
@ES We report the following results:
@EN 1. In cases such as juxteaarticular fracture where fixation is difficult, the rigid fixation was possible with a modified usage of monofixator.
2. In plafond fracture of ankle, its treatment was based on AO principle but when the comminution is severe or a high risk of infection or soft tissue problem is expected, we used the limited internal fixation and external fixation. Thus, we were

avoid the soft tissue damage and infection.
We also obtained a rigid fixation which resulted in an early weight bearing and ROM exercise. Therefore, joint contracture and osteoporosis could be prevented.
3. In certain circumstances where the joint fusion became necessary during the treatment process, the initially applied monofixator could be continuously used without removing or changing the monofixator.
4. Even in cases of nearly amputated injury or severe osteomyelitis where amputation might be needed, we were albe to apply the monofixator for the pupose of limb salvage opera-tion.
